Health Forms

Scouts must sign their existing health forms. They are good for three years as long as they are updated and signed annually before summer camp.

Boys and adults requiring new health forms need to make two copies for the two books that the troop must keep.

Adults going to summer camp also need to provide health forms. Those over age 40 need new forms every year.



Leadership Application Forms

Leadership Position Application
The Scouts may apply for a Leadership Position with this form. To Apply for a Position, print and fill-out this form then turn-in prior to Troop elections.

Patrol Attendance Form
Patrol leaders use these forms to take attendance at weekly Troop Meetings.

Campfire Planning Worksheet
Use this form to plan your campfire programs. It contains instructions, samples, and space to gather your information

Campfire Program Form
This is used as to map out the final campfire agenda

Troop Meeting Planning Form
This is used by the SPL/ASPL to plan Troop Meetings
